Lage des Sinus cavernosus

Der Sinus cavernosus ist ein erweiterter Venenraum in der Hirnhaut (Dura mater) an der vorderen Schädelbasis, einige Zentimeter oberhalb und hinter der Augenhöhle (Orbita) gelegen. Die "Gekrümmte Höhle" oder auch Felsenbeinhöhle wird von arteriellen und venösen Blutgefäßen und Nerven durchzogen, die in zahlreiche Nervenfasern einmünden. Ist rechts wie links vorhanden.

Nach der bisherigen Vermutung erweitert sich bei der Clusterattacke in dieser Region die obere Halsschlagader (Arteria carotis interna), die dadurch in dem umgebenden Nervengeflecht (Plexus Carotis) die Schmerzen auslöst. Durch die mitbetroffenen Nervenfasern, die in die zugehörige (ipsilaterale) Gesichtshälfte führen sind die Begleitsymptome wie Augentränen etc. erklärbar. Die angrenzenden Schädelknochen gewähren den erweiterten Gefäßen keine Ausweichmöglichkeit, was den Druck auf die Nerven und damit die Schmerzen verstärke. Nach heutigem Kenntnisstand ist die Gefäßerweiterung eine Folge der Schmerzen und nicht die Ursache.[1][2][3][4]

Als Ursache des Cluster-Kopfschmerzes ist der Hypothalamus in den Fokus der Wissenschaft gerückt.
